Will my downloads still be on the card?−

Yes. The card is untouched. Once the reader is replaced the console sees the games again.

Could the card itself have failed?+

If it is not readable in a PC either, the card is the problem rather than the slot. Check that first if you can.

Do I need an appointment to drop off?+
